How to Prepare

No fasting is strictly required but fasting for 4–6 hours improves accuracy of calcium and phosphate results. Inform the doctor of all medications including steroids (which accelerate bone loss), calcium supplements, and hormone replacement therapy. Note any previous fragility fractures (fracture from minor trauma or fall). Bring details of your menopause history if applicable.

Who needs a bone density test in Malaysia?
DEXA bone density scanning is recommended for all women aged 65 and above, post-menopausal women under 65 with risk factors, men aged 70 and above, anyone who has had a fragility fracture, those on long-term steroid therapy, and patients with conditions causing bone loss (rheumatoid arthritis, thyroid disorders, malabsorption). Our doctors use the FRAX calculator to identify who needs urgent referral.
Can blood tests diagnose osteoporosis?
Blood tests (calcium, Vitamin D, PTH, ALP) assess bone metabolism and identify secondary causes of bone loss — they do not directly measure bone density. Osteoporosis diagnosis requires a DEXA scan. However, our blood panel identifies Vitamin D deficiency and secondary causes of bone loss that can be treated before DEXA, improving the overall outcome.
How is osteoporosis treated once diagnosed?
Treatment depends on DEXA T-score and FRAX fracture risk. Mild bone loss (osteopenia) may be managed with calcium and Vitamin D supplementation, weight-bearing exercise, and fall prevention. More severe osteoporosis or high fracture risk needs bisphosphonate medication (alendronate, risedronate) prescribed by a doctor.
