Pleural Effusion (Fluid Around the Lung) Treatment in Johor Bahru

Efusi Pleura (Cecair di Sekeliling Paru-paru)

A pleural effusion is an abnormal accumulation of fluid in the pleural space — the cavity between the lung and the chest wall. The pleural space normally contains only a tiny amount of lubricating fluid. When excess fluid accumulates, it compresses the lung and impairs breathing. Pleural effusions are classified as transudates (caused by imbalanced fluid pressures — heart failure, liver cirrhosis, kidney disease) or exudates (caused by local pleural pathology — infection, malignancy, tuberculosis). In Malaysia, tuberculosis is a particularly important cause of exudative pleural effusion and must always be considered.

Symptoms of Pleural Effusion (Fluid Around the Lung)

Progressive shortness of breath, especially on exertion
Chest pain that may be pleuritic (sharp, worsening with breathing)
Dry cough, particularly when lying down
Reduced breath sounds on the affected side
Dullness to percussion over the fluid area
In large effusions: significant breathlessness even at rest and cyanosis

⚠️ When to See a Doctor

Seek urgent medical assessment for any significant shortness of breath, especially with chest pain, fever, or weight loss. A chest X-Ray or ultrasound can immediately confirm the presence of pleural effusion. Do not delay as large effusions can cause respiratory failure, and infectious causes (empyema, tuberculosis) require prompt treatment.

Treatment at Klinik Muhibbah

At Klinik Muhibbah, Masai, our doctors perform clinical assessment and arrange chest X-Ray to identify pleural effusion. Blood tests (FBC, CRP, LFT, RFT, echocardiogram referral) are arranged to identify the cause. Large symptomatic effusions require urgent hospital referral for thoracocentesis (pleural fluid drainage). TB-related effusions require full TB treatment. We coordinate urgent specialist referrals. Prevention Tips

1Manage heart failure, liver cirrhosis, and kidney disease optimally to prevent transudative effusions
2Complete full tuberculosis treatment if diagnosed — incomplete treatment leads to resistant TB and recurrent effusions
3Seek early treatment for pneumonia to prevent progression to empyema (infected pleural effusion)
4Get annual influenza and pneumococcal vaccination to reduce risk of serious chest infections
5Attend regular follow-up if you have a known cause of pleural effusion such as heart failure or malignancy

Frequently Asked Questions

Is pleural effusion serious?
Yes — pleural effusion is always a significant medical finding requiring investigation for the underlying cause. Small effusions may not cause symptoms, but moderate to large effusions compress the lung causing breathlessness. The seriousness depends entirely on the underlying cause — ranging from treatable heart failure to malignancy. Urgent assessment is needed. Come to Klinik Muhibbah or seek emergency care for significant breathlessness.
Can tuberculosis cause fluid around the lung?
Yes. Tuberculous pleural effusion is one of the most common causes of exudative pleural effusion in Malaysia. It occurs when TB bacteria infect the pleural space, causing an inflammatory exudate. Characteristic features include lymphocyte-predominant fluid, elevated ADA (adenosine deaminase), and positive TB test. Full TB treatment is curative. We assess for TB at Klinik Muhibbah, Masai.
How is pleural effusion treated?
Treatment depends on the underlying cause and size. Small effusions from heart failure often resolve with diuretics. Large or symptomatic effusions require thoracocentesis (needle drainage) by a specialist. Empyema (infected effusion) requires chest drain insertion. Malignant effusions may require pleurodesis. Our doctors at Klinik Muhibbah assess, arrange imaging, and refer urgently where needed. Book at movo-x.com/kiosk/muhibbah.
