The parotid glands are the largest salivary glands, located in front of and below each ear. Parotid swelling can be unilateral (one side) or bilateral (both sides), acute or chronic. The most well-known cause is mumps (viral parotitis) though vaccination has significantly reduced this. In adults, the more common causes are parotid stones (sialolithiasis), bacterial parotitis, Sjogren syndrome, parotid tumours (both benign and malignant), and sarcoidosis. HIV-associated parotid enlargement is also seen in Malaysia. Parotid swelling warrants medical assessment to identify the cause and exclude malignancy.

Symptoms of Parotid Gland Swelling

Swelling in front of or below the ear — unilateral or bilateral
Pain or tenderness over the swollen gland, worse when eating
Reduced saliva flow or dry mouth
Pus or cloudy saliva from the parotid duct opening in bacterial infection
Fever and feeling unwell in acute bacterial parotitis or mumps
Persistent painless firm swelling in tumour or chronic condition

⚠️ When to See a Doctor

See a doctor for any parotid swelling that persists more than 2 weeks, is rapidly enlarging, is associated with facial nerve weakness (inability to close the eye or drooping mouth on one side), has overlying skin changes, or is firm and fixed. Unilateral persistent parotid swelling in adults must be assessed to exclude parotid tumour.

Is parotid gland swelling serious?
It depends on the cause. Acute viral or bacterial parotitis is usually treatable and resolves completely. Can a blocked salivary duct cause cheek swelling?
Yes. Salivary gland stones (sialolithiasis) block the parotid duct causing painful swelling that is worse during eating (when saliva production increases). Is mumps still a cause of parotid swelling in Malaysia?
