Why Sexual Health Matters — And Why People Avoid It
Sexual health is an integral part of overall health, yet it is one of the most under-addressed areas in Malaysian primary care. Stigma, embarrassment, and cultural sensitivities cause many people to avoid testing even when they have concerns. The result is that sexually transmitted infections (STIs) are diagnosed late, when complications are more likely and transmission chains are longer. Common STIs in Malaysia include HIV, syphilis, gonorrhoea, chlamydia, genital herpes, HPV (human papillomavirus), and hepatitis B. Many of these infections are entirely asymptomatic — meaning you can have an STI without any symptoms, and unknowingly transmit it to partners. The only way to know your status is to get tested. Common STIs: Symptoms and What to Look For
HIV: often asymptomatic for years after initial infection. Early infection may cause a flu-like illness (fever, rash, swollen lymph nodes) 2-4 weeks after exposure. Regular testing is the only reliable way to know your status. Syphilis: presents in stages. Primary stage — painless sore (chancre) on genitals, anus, or mouth. Secondary stage — rash on palms and soles, flu-like symptoms. Easily treated with antibiotics if caught early. Gonorrhoea: burning urination, discharge from penis or vagina, or no symptoms at all (especially in women). Chlamydia: often completely asymptomatic, especially in women. Can cause infertility if left untreated. Burning urination and discharge are possible. Genital herpes (HSV-2): painful blisters or sores on genitals. Can be managed but not cured — antiviral medication reduces outbreaks. HPV: often no symptoms. Certain strains cause genital warts; others (high-risk HPV) cause cervical cancer (screened via Pap smear). Hepatitis B: jaundice, fatigue, abdominal pain in acute phase. Chronic infection is often asymptomatic but damages the liver over time.

HIV Testing and PrEP Awareness
HIV (Human Immunodeficiency Virus) testing is available at Klinik Muhibbah and is entirely confidential. There are two types of HIV tests: rapid test (results in 20-30 minutes from a finger-prick or blood sample) and laboratory ELISA test (more sensitive, results in 1-3 days). The window period (time between exposure and detectable antibodies) is typically 18-45 days for modern 4th generation tests — retesting at 90 days gives definitive results for most exposures. PrEP (Pre-Exposure Prophylaxis) is a daily medication (tenofovir/emtricitabine) taken by HIV-negative individuals at high risk of HIV exposure. When taken consistently, PrEP reduces HIV risk by over 99%. It is recommended for men who have sex with men, people with HIV-positive partners, and individuals with multiple partners. PEP (Post-Exposure Prophylaxis) is emergency medication taken within 72 hours after a potential exposure — it must be started as soon as possible. Who Should Get Tested and How Often
The recommendation is: anyone who is sexually active should know their STI status. More specifically, annual testing is recommended for sexually active adults under 25; anyone with a new partner; anyone with multiple partners; those with a partner who has multiple partners; men who have sex with men (more frequent testing — every 3-6 months is often recommended); sex workers; and anyone who has had unprotected sex with a partner of unknown status. Testing for the full STI panel includes HIV, syphilis (VDRL/RPR), hepatitis B (HBsAg), gonorrhoea and chlamydia (urine or swab), and herpes (clinical assessment or blood test). You do not need symptoms to request STI testing. Simply inform the doctor that you want a sexual health screen and they will recommend the appropriate tests based on your history and risk factors.

However, condoms do not protect against STIs transmitted through skin-to-skin contact like herpes and HPV.
