The ABO and Rh Blood Group Systems
Your blood type is determined by two systems: ABO (A, B, AB, or O) and Rh factor (positive or negative). Together these give your full blood type: A+, A-, B+, B-, AB+, AB-, O+, or O-. The ABO system is determined by antigens on the surface of your red blood cells. Type A: A antigens on red blood cells, anti-B antibodies in plasma. Type B: B antigens, anti-A antibodies. Type AB: both A and B antigens, no antibodies — universal recipient. Type O: no ABO antigens, both anti-A and anti-B antibodies — universal donor for red blood cells. The Rh factor (Rhesus factor) refers to the D antigen. If you have it, you are Rh positive (+). If not, you are Rh negative (-). Approximately 92–95% of Malaysians are Rh positive. In Malaysia, the most common blood types are O+ (approximately 35–38%), A+ (approximately 25–27%), and B+ (approximately 30–32%). AB is the rarest group.

Blood Type and Transfusions: Compatibility Rules
Knowing your blood type is critical in emergency situations requiring blood transfusion. Receiving incompatible blood triggers a severe immune reaction that can be fatal. Compatibility rules for red blood cell transfusion: O- is the universal donor — can give to all blood types (used in emergencies when type is unknown). AB+ is the universal recipient — can receive from all blood types. For non-emergency transfusions, hospitals always cross-match blood to ensure exact compatibility. For plasma transfusions (a different component), the compatibility rules are reversed: AB plasma is the universal plasma donor. In Malaysia, blood is obtained from voluntary donors at Hospital Sultanah Aminah blood banks and National Blood Centre (Pusat Darah Negara). Blood O+ and B+ are always in high demand due to their prevalence in the Malaysian population. Rh Factor and Pregnancy: Why It Matters
The Rh factor has particular importance during pregnancy. If you are Rh negative and your baby inherits Rh positive blood from the father, your immune system may detect the baby blood as foreign and produce antibodies against it. This condition is called Rh incompatibility. In a first pregnancy, this rarely causes problems because the immune response typically develops late in the pregnancy or during delivery. However, in subsequent pregnancies with an Rh positive baby, the pre-formed antibodies can cross the placenta and attack the baby red blood cells, causing haemolytic disease of the foetus and newborn (HDFN) — a potentially serious condition causing anaemia and jaundice. Prevention: all Rh negative pregnant women are given anti-D immunoglobulin (Rh immunoglobulin) injections during pregnancy (typically at 28–34 weeks) and within 72 hours of delivery of an Rh positive baby. Blood Type and Organ Donation in Malaysia
Malaysia has one of the lowest organ donation rates in Asia. As of recent statistics, fewer than 1 in 1,000 Malaysians are registered as donors, compared to rates exceeding 40 per million in developed countries. Blood type is a key factor in organ transplant compatibility — donor and recipient typically need matching or compatible blood types (with some exceptions for certain organs). Major blood type compatibility for solid organ transplants largely follows similar rules to blood transfusion. How to register as an organ donor in Malaysia: register online at www.dermaorgan.gov.my or at any government hospital. Carry your donor card or note it in your MyKad records. Inform your family of your wishes — family consent is required for organ donation to proceed in Malaysia even if you are a registered donor. Blood type does not prevent you from donating blood or organs — all types are needed. Type O donors are particularly valuable as universal red cell donors, while AB plasma donors are universally applicable for plasma donations.
