First Trimester (Weeks 1 to 12): The Booking Visit and Early Care

The first antenatal visit — often called the booking visit — should ideally take place between weeks 8 and 12. This is the most information-rich visit of your entire pregnancy. Pregnancy confirmation and dating: a urine or blood hCG test confirms pregnancy. Your Expected Date of Delivery is calculated from your Last Menstrual Period. If your periods are irregular, a dating ultrasound at 8 to 12 weeks gives the most accurate date. Medical and family history: your doctor documents your medical history, family history, previous pregnancies, current medications, and lifestyle factors. Booking blood tests — these are essential and should not be skipped: - Blood group and Rhesus factor: Rh-negative mothers need monitoring and in some cases anti-D injections to prevent haemolytic disease of the newborn - Full Blood Count: screens for anaemia, very common in Malaysian pregnancies - VDRL: congenital syphilis is preventable with treatment - HIV screening: if positive, interventions dramatically reduce the risk of mother-to-child transmission - Hepatitis B surface antigen: babies of HBsAg-positive mothers need vaccination at birth - Thalassaemia screening: critical in Malaysia where carrier rates are approximately 5% - Rubella immunity: if not immune, vaccination post-delivery is recommended Prescriptions at the booking visit: folic acid (400 to 800 mcg per day, ideally started before conception and continued until week 12), iron and folic acid combination supplement, and sometimes vitamin D. First ultrasound: confirms intrauterine pregnancy location, confirms heartbeat and viability, and establishes accurate gestational age. A nuchal translucency scan at 11 to 14 weeks can screen for chromosomal abnormalities like Down syndrome.

Second Trimester (Weeks 13 to 28): The Anatomy Scan and OGTT

The second trimester is often described as the most comfortable phase of pregnancy — morning sickness usually eases and energy improves. Two critical tests occur in this trimester. Antenatal visits (monthly, weeks 13 to 28): each visit includes blood pressure measurement for pre-eclampsia screening, urine dipstick test checking for protein and glucose, fundal height measurement checking baby's growth, fetal heartbeat check with Doppler, and weight measurement. Anatomy scan at weeks 18 to 22: also called the anomaly scan, this is the most detailed ultrasound of the pregnancy. It takes 30 to 45 minutes and systematically checks brain and spine structure; heart chambers and outflow tracts; abdominal organs; limbs and digits; face for cleft lip screening; placental position and amniotic fluid volume; and cervical length which screens for preterm labour risk. Approximately 50 to 70% of major structural abnormalities can be detected at this scan. OGTT (Oral Glucose Tolerance Test) at weeks 24 to 28: screens for gestational diabetes (GDM), which affects approximately 25% of Malaysian pregnancies — one of the highest rates in Asia, driven by diet, BMI, and genetic factors. The test involves fasting overnight, a fasting blood sugar check, drinking 75g of glucose solution, then blood sugar checks at 1 and 2 hours. GDM is diagnosed if fasting glucose is 5.1 mmol/L or above, 1-hour is 10.0 or above, or 2-hour is 8.5 or above.

Third Trimester (Weeks 29 to 40): Final Preparation and GBS Screening

The third trimester brings increased monitoring frequency as delivery approaches. Visits move to fortnightly from week 28 and weekly from week 36. Weeks 29 to 36 visits: increased monitoring of blood pressure and urine, fetal growth assessment, baby's position in the uterus checking for breech presentation, repeat full blood count as anaemia is common in the third trimester, and glucose monitoring if you had GDM. Group B Streptococcus (GBS) screening at weeks 35 to 37: GBS is a bacterial infection that approximately 20 to 30% of women carry harmlessly in their vaginal flora, but which can cause serious infection in newborns during delivery. A vaginal swab is sent to the laboratory. GBS-positive mothers receive intravenous antibiotics during labour to protect the baby. Pre-delivery counselling: discuss the birth plan, preferred delivery method, what to expect in early labour, signs that you should go to hospital (regular contractions every 5 minutes, water breaking, heavy bleeding, reduced fetal movement), and post-partum contraception. Fetal kick counting from week 28: you should feel at least 10 movements within 2 hours. If you notice a significant reduction in fetal movements, contact your clinic or go to hospital immediately. 4D Ultrasound: Seeing Your Baby Before Birth

Standard 2D ultrasound gives flat, grey-scale cross-sections of your baby. 4D ultrasound adds the dimension of time — creating a real-time moving image of your baby in three dimensions, allowing you to see facial features, expressions, yawning, thumb-sucking, and movements in unprecedented detail. Best timing for 4D ultrasound: weeks 26 to 32 offer the best combination of image quality and detail. Before week 26, there is not enough subcutaneous fat for detailed facial definition. After week 34, the baby's head often descends into the pelvis and may be face-down, reducing visibility. Warning Signs During Pregnancy

Most pregnancy symptoms are normal, but certain warning signs require immediate medical attention. Normal and manageable symptoms: - Nausea and vomiting in the first trimester: eat small frequent meals, avoid strong smells, try ginger tea. If you cannot keep any fluids down, seek medical help. - Fatigue: rest as much as possible, especially in the first and third trimesters. - Round ligament pain: sharp, brief pains on the sides of the lower abdomen with movement. Normal from the second trimester as the uterus expands. - Mild swelling of feet and ankles: normal in the third trimester, especially in Malaysia's heat.
